Supported Community Living Coordinator


The North Iowa Transition Center (NITC), a progressive and compassionate organization serving adults with mental illness, is seeking a Supported Community Living Service Coordinator to support adults with mental illness in developing greater independent living skills and involvement in the community. This position reports to the Supported Community Living (SCL) Administrator. The schedule for this position is typically Monday - Friday, 8 am - 5 pm.


  • Coordinate and facilitate the Individual Service Plan (ISP) team meetings.
  • Ensure consumers complete program assignments according to the ISP.
  • Assess for issues of concern with consumers or with operational problems. Promptly report these issues to the SCL Program Director.
  • Assist and coach consumers in developing rehabilitation skills in areas including, but not limited to: personal hygiene, housekeeping, budgeting, laundry, meal planning and preparation, medications management, transportation, and social/recreational activities.
  • Teach basic safety and emergency procedures, including reviewing crisis plans and symptom management techniques.
  • Advocate for consumer rights and facilitate effective use of services through consumer choice.
  • Ensure that all appointments are made and kept (i.e., medical exams, psychiatric appointments). Document reasons if this did not occur.
  • Ensure that consumer records are kept in accordance with SCL licensure requirements including, but not limited to: admission documentation, service notes, physician consults, social history addendum, and releases of information.
  • Coordinate the consumer’s treatment with other key personnel including case managers, mental health advocates, physicians, family members, and other social service agencies.
  • Complete other duties as assigned. The above duties are illustrative and are not all inclusive for the position.

A Bachelor's Degree in a social services related discipline (social work, human services, psychology, etc.) is required. Experience with the delivery of services to persons with behavioral health concerns in the community is preferred. A valid Drivers License & clean driving history for the past 5 years is also required.
